Prescription Synchronisation

Many of our usual procedures have been amended in response to the current Coronavirus pandemic. Please check our Coronavirus Updates page for the most up to date guidance.

If you would like to save time and order all the medication you have on repeat at the same time rather than irregular times throughout the month please use this form.

When you next need to request an item on repeat count up all the tablets you have and fill in the form for each medication you request. We can then issue a “one off” prescription of the tablets you need to synchronise all your medication to within a day or two.

Please detail the names and strengths of medication which need synchronising, please include the dose you are taking and the number of tablets you have left.

For example:

Metformin 500mg twice a day 14 tablets left – this allows us to see you have 7 days of medication left.

This should be completed for all medicines taken on a regular basis to allow us to accurately synchronise all medication effectively.

Please allow 3 working days before collecting your prescription.

If you have not done so already we recommend registering for our Online Services system which will remember which medications you are on and makes requesting repeat prescriptions faster and easier.

Please do not use this service for any urgent medical queries as this service is only monitored during practice working hours on a daily basis.

Please note: If you have an urgent medical query you should telephone the surgery or contact the out of hours service by calling 111.
