NHS Care Data Opt-Out

The NHS has commissioned the Health & Social Care Information Centre (HSCIC) to extract personal confidential patient data to enable them to plan and improve services and compare care in different areas. GP Surgeries have no choice but to allow the HSCIC to extract this information.

This health information along with details such as your date of birth, postcode and NHS number but not your name, are sent to a secure system where it can be linked with other health information.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything.

If you do not wish information about you to be used in this way, please complete the opt-out form below and return the completed form to reception. Please note, your access to healthcare and the care that you receive will not be affected by either decision.
