We can only accept patients who are within our boundary area
To find out if your address is within our practice boundary please put your postcode into the searchbar in the top right hand corner of the map frame
If you would like to register with the practice please complete the Pre-register online form.
Acceptance on to the list is as at the discretion of the Surgery and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen surgery, for example you may live too far away. If this is the case simply choose another surgery in your local area.
We have the right to refuse to register patients if they have a violent flag against their record.
Pre-register as a new patient online
Step 1: Complete New Patient Registration form online
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services when you are fully registered.
We will send you a text message once your registration has been completed with a link containing information on how to make the most out of the Practice.
Please complete one form for each family member.
Step 2: Upload scans of photo identity and proof of address
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online during the COVID pandemic. Photo ID and proof of residency is not required for patients under the age of 18.
Step 3: Set Up Required Repeat Medications
Repeat prescriptions are not automatically set up as part of a medical record transferring from a previous practice and need to be requested and agreed with a MMG GP.
Book a New Patient Medication Review with our Pharmacy Team at the earliest opportunity and before your current medication runs out.

- Click on the Start Now button, top right just under our logo
- Click that you are registered and continue
- Click on the box called Medication requests/queries
- Then select the box called I have been asked to book a medication review
If a new patient requires medication urgently whilst awaiting a medication review or set up of online services then please complete the ‘Request repeat medication’ sub-tile under the ‘Repeat Prescriptions’ Klinik button below.
Step 4: Register for Online Services
You can now access a variety of services online without having to phone or visit the surgery. Sign up now to GP online services and you will be able to use a website or app to:

- Access your medical records
- Accessing test results
Our online services are quick, easy and convenient to use from a smartphone, tablet or computer.
They will save you time and help you take more control of your health, particularly if you have any long-term medical conditions which require regular monitoring and prescriptions.
