How to Register with the Surgery

If you have recently come to live in Marlow or the surrounding area and are eligible to register with the surgery you can complete the attached forms and bring them in to the surgery to register. Alternatively, you can collect the registration forms from any of our surgeries. You can refer to the map below to see whether your address is in our catchment area.

Download, print and complete the following Registration Forms:

We require two forms of ID which you must present in person - one for proof of identity and one for proof of address. All forms of ID must be valid and proof of address must be no more than three months old.

Notification from Home Office

Please print off and complete the GMS1 Registration Form and New Patient Health Questionnaire and bring them in to the surgery together with your proofs of ID. In order to process your application efficiently, we advise you to obtain your NHS number which you can get from your previous GP.

For all children under the age of 16, we also require you to complete an Immunisation Form which enables us to enrol your child with the appropriate immunisation programme where necessary. We do not require ID for children who are existing NHS patients.

We would advise you to bring in your completed forms and ID between 1pm and 3pm when the surgery is less busy. Unfortunately, we are unable to accept new registrations during extended hours or Saturday surgeries. If you require an appointment we would ask you wait 48 hours if possible to allow the registration to be processed.  If you require an emergency appointment, you may be seen as a temporary patient in the meantime.

If you are registering with the NHS for the first time and do not currently have an NHS number, you will need to provide evidence of your intention to remain in the UK. For EEA residents you will need to produce evidence in the form of a work contract or long term rental agreement etc. For non-EEA residents, including children, we require your UK Entry Clearance Visa or letter from the Home Office.

Please note that every care has been taken in the preparation of information on this Postcode Mapping Tool, however there may be instances where the Postcode Mapping Tool does not include your location even though it is within our Practice Boundary.
